At 21st Century Rehab, our therapists work closely with you, your family, and your referring physician to design an individualized treatment program to maximize function and regain your independence following an injury, surgery, or illness.
Our licensed therapists have extensive training and attend continuing education courses each year to provide the most current and effective treatment techniques.
In the state of Iowa, a referral from a physician is not required to receive a physical therapy evaluation or treatment. Some exceptions do occur. Medicare and Medicaid always requires a referral to obtain physical therapy services. This referral may come from a physician, a physician assistant, a nurse practitioner, a dentist or podiatrist. In addition, some insurance companies require a referral to provide insurance coverage. You should call your insurance company to determine whether or not you are required to have a referral for coverage.

Your First Appointment:
At your first appointment, your therapist will obtain a thorough history of your medical condition and perform an evaluation, discuss a treatment plan, and set goals with you.
Financial Policy/Billing Information:
21st Century Rehab accepts Medicare, Medicaid, HMO/PPO Insurances, Workers' Compensation, and Auto Insurance Plans. As a courtesy to our patients, we will verify your benefits before your first visit and will file charges to your insurance company. However, it is your responsibility to know your insurance policy and its limitations. You will be responsible for any co-pays, deductibles, or non-covered services set by your insurance policy.
We accept cash, checks and credit cards (MasterCard and Visa) for payment, and offer a self-pay rate for patients without outpatient therapy benefits or without insurance.
